Dear Jamie,

It is my great pleasure to offer you a full-time position as a Case Manager with Home Care Assistance.  Your start date will be on Monday, Sept 6, 2021.

The attire is business casual. (Appropriate business casual dress typically includes slacks, blouse, a dress or skirt at knee-length or below, optional tailored blazer, knit shirt or sweater, and shoes that cover most of the foot.)

Your typical job functions will include but are not limited to:

Conducting assessments with prospective clients and setting up the relationship for success:

Addressing their questions and overcoming their obstacles in order for them to appreciate our value and convert to Clients

Getting them to sign our Client consent form and provide a method of payment

Facilitating caregiver meet and greets when requested

Participating in caregiver orientation for complex Clients

Developing comprehensive, goal-oriented care plans to help set our caregivers up for success

Providing active care management for our clients. Active care management includes:

Regular home visits plus any additional home visits in between for any significant change in condition and hospital visits for any Clients that are recently hospitalized

Writing detailed notes in our system for all visits conducted to document observations, recommendations and next steps

Updating family members after each home visit on any changes, recommendations, concerns, etc.

Serving as the main contact for any major issues involving the Client or caregivers assigned to your Client

Management/regular updates of our care plans, when there are any significant changes in condition or new directives

Attending family meetings, case conferences and/or important doctor’s appointments, as required to advocate for the Client

Collaborating with any health care professionals involved (e.g. director of care at facility, doctor, palliative care team, mental health team, etc) to ensure coordinated care

Referring Clients/families to other health care professionals or resources in the community that can provide additional support to the Client

Developing any functional and cognitive reports/summaries needed for doctor’s appointments

Working closely with Operations Manager to ensure Client satisfaction and retention

Working with Schedulers to ensure correct caregiver matching and that all shifts are filled

Implementing our Home Care Assistance operating model

Building and representing the strength of our brand across the entire community by:

Attending trade shows, aging fairs to represent our company

Promoting our services to potential referral sources that you organically come across (e.g. social workers at the hospital, directors of care at the residential facilities) and building on the relationships

Attending business development meetings/presentations to help promote the business

Providing after-hours support for our clients, referral partners and prospective clients (including evenings and weekends) - 1 day of weekday support/week (before and after office hours and 1 weekend/month.

In addition, you have agreed to the following terms:

You will work out of the West Vancouver office

We will provide you with a cellphone and you will give that number out to your Clients and caregivers so they can reach you directly

You will attend status meetings every Monday or Friday either in person or over the phone

You will have weekly check-ins with the Operations Manager to go over your Clients and will regularly update her on your Clients to ensure a seamless workflow

You will attend our bi-weekly Case Management meetings either in person or over the phone

For your role, you will receive $68,000/year.

Additional benefits:

You will receive a Performance Evaluation after 90 days which will be reviewed by you and your manager.

You will also be receiving annual reviews which may involve a salary increase, if you received a positive review

You will be paid bi-weekly.

You are entitled to two weeks of vacation and 5 personal days per year

You will be provided with a cell phone and data plan for business use

We will reimburse you up to $250/month for your gas
