Private and Confidential May 17, 2022 Jay Bradley New Hire Address1 New Hire Address2 Postal Code Dear Jay, On behalf of Home Care Assistance of Vancouver, I am pleased to extend you an offer of employment as a Full Time, exempt position as Home Care Liaison (HCL). You will be reporting to Matt Vijayan, Sales Director of Canada. The effective start date for you in this position is May 24, 2022, on the following terms and conditions: Compensation Your annual base salary will be $110,000 and will be paid to you on a bi-weekly basis. All compensation is subject to statutory deductions as well as deductions you may authorize for benefit plans and programs, if eligible. Group Insurance Benefits As a full-time employee you will be eligible to participate in the company’s benefit package the after the probationary period. Further information on the company’s benefits program is attached for your review. Vacation Entitlement The right to vacation shall be acquired for a period of 12 months which will be prorated for the first year, referred to as the base year from your start date which is May 24, 2022. The amount of annual vacation pay represents 6% (three weeks) of the wage earned during the reference year for the vacation of the year following the reference year. Other Incentives Vehicle Stipend - As a full time employee you are entitled to a vehicle stipend of $650/month. Commission - if eligible, will be based on the Home Care Liaison Commission structure. Probation The company views the first 90 days of employment as a probationary period. Policies In joining the organization, you agree to adhere to all HCA policies, rules, systems, and procedures as shall be in place. The organization reserves the right to make changes to its policies, rules, systems and procedures at its own discretion. Terms and conditions The terms and conditions set out above and in the employment agreement constitute the entire agreement terms of your employment relationships with us and you acknowledge and agree that you are replying on no representations or promises with respect to your employment other than outlined herein. As required by Home Care Assistance, this offer is contingent upon the successful completion of a background investigation, which includes a criminal background check, professional references, prior employment verification, and any other requirements of the company and or Province/City. Additionally, as service providers to a high risk population, HCA recommends that all internal corporate and field office employees be fully vaccinated against Covid-19. If required the company may request you to provide proof of vaccination prior to hire. Please inform us if you have any medical or lawful religious exemption and you will be provided with reasonable accommodation. Please note that the organization reserves the right to organize its operations and workforce to best meet business needs. As such, the organization may, from time to time and at its sole discretion, change or alter your duties, responsibilities, reporting relationships, schedule, compensation, benefits, policies, incentive plans or transfer within the organization, or any other term or conditions of your employment, without notice, except if and as required by the Employment Standard Act. Our employment relationship is ‘at will’ and you have the right to terminate the employment relationship at any time, subject only to the agreements described above. While we hope you will be successful with Home Care Assistance, we also reserve the right to terminate our employment relationship at any time.
