Home Care Assistance (‘HCA”) has been notified of your violation of your Client Consent Agreement (“Agreement”) with HCA.

The Agreement makes clear that a client is required to pay HCA a “one-time placement fee” of $10,000 in the event a client wishes to employ an HCA caregiver within a three (3) year period after caregiver’s termination of employment with HCA.  Both you and our caregiver [insert CG name]
