Weeks 1-3 Research and Set Up

Clients who live in an AHS-funded facility don’t qualify

Independent living facilities do qualify

Dennis – to have a one-page electronic document to follow up with clients when they call about CDHC

Additional Brochures are to be handed out to all hospitals

Program funded through Blue Cross

AHS – Two to four weeks for an Assessment and a referral from a doctor or through transition services.

Target doctors as they can give a referral for home care – physician brochure

CDHC takes 1 to 15 days to begin to use the program if the client is already on AHS public care

The current cost for reimbursement is $32.95 (clients will pay more due to the complexity of care and the more affluent client). Matching the price for low-income clients. (this will be at a 44% profit margin with a $18.50 caregiver to match the provincial reimbursement.)
