Please complete this form if you are receiving care that is paid for by an applicable Canada government/municipality program.

This is to certify that the Care Recipient named below is currently receiving care from TheKey that is paid for by an applicable Canada government/municipality program and therefore the services are exempt from Harmonized Sales Taxes (“HST”). In the event Care Recipient no longer is participating in the applicable government/municipality program, Care Recipient is required to immediately notify TheKey in writing and TheKey will subsequently charge HST on invoices as required by the Canada Revenue Agency.
