By signing below, you acknowledge that you have reviewed and understand the above Caregiver ‘Always and Never’ statements issued by Kendal at Home and agree to follow these statements while providing care to Kendal at Home members. If you have any questions, you can reach the Kendal at Home Care Coordinator on-call 24/7 at 440-960-4804. ALWAYS | NEVER | DEFINITIONS 1.Abide by all policies and code of conduct set forth by your agency. | Stray from the rules and code of conduct outlined by your agency. | *Your Agency refers to the organization with which you are directly employed. 2.Be professional and respectful and expect the same from others. | Be rude, impatient, or offensive or share personal information. | *Personal Information refers to hardships/struggles, pay rate, religious and political beliefs and any other information not directly related to the care of the member. 3.Review the care plan in detail and ask for clarification when needed. | Perform tasks or duties beyond your area of expertise/scope of practice. Partake in heavy cleaning. | *Duties beyond expertise/scope of practice refer to those tasks requiring special certification or training ie: wound care, medication management, Hoyer lift, etc. Caregivers are not permitted to perform heavy cleaning, such as scrubbing floors, moving furniture, climbing on ladder (refer to your agency policies) 4.Accept a member’s verbalized gratitude and follow your agency’s gifting policy. | Accept gifts or money from the member or family. | *Agency’s gifting policy refers to that established by your organization of employment. 5.Report changes in member’s health or status immediately to your supervisor to be relayed to Kendal at Home. | Consider a change in a member’s status too small to report. | *Changes in member’s status refers to physical or emotional/mental health or medical changes and includes falls with or without injury. 6.Report any concern for abuse, neglect, or exploitation of a member that you witness or expect. | Refrain from reporting suspected abuse, neglect, or exploitation of any kind. | *Abuse, neglect, or exploitation of a member may include physical, mental/emotional, or verbal abuse or neglect and exploitation of any kind, including financial. 7.Complete documentation each shift. | Finish shift without documenting all required activities and information. | *Documentation refers to completion of any notes and/or checklists either electronic or on paper that is required by your agency or Kendal at Home. 8.Maintain a professional relationship with member/family. | Socialize with, call, or visit the member or their family outside of scheduled shift. | *Professional relationship refers to the relationship required to provide scheduled care for a member while adhering to all agency policies and procedures and does not include personal or intimate friendships or relationships. 9.Maintain a safe environment that allows for provision of care. | Fail to report safety or environmental issues that may affect the member’s care. | *Safe environment refers to free of clutter, debris, or any other safety/health hazard. 10.Keep cell phones silenced and use only during emergencies. | Use cellphones for non-emergency purposes. | *Non-emergency purposes refer to games, email, social calls, social media, texting, etc. 11.Remind members that caregivers are unable to be involved in legal paperwork or management of finances. | Sign or prepare any legal paperwork or conduct banking transactions of any kind on the member’s behalf. | *Preparing legal paperwork may include wills, trusts, DNR, POA, DPOA, living will, etc. Management of finances refers to financial paperwork or banking transactions, balancing checking account, paying bills, writing checks, or assisting in any other financial matters. 12.Schedule appropriate childcare during your work shift as needed. | Bring children to work or allow personal visitors during scheduled shifts. | *Personal visitors refer to friends and/or family members of the care giver. 13.Work for members only through the agency for which you are employed and during shifts approved by Kendal at Home. | Work privately for a member of Kendal at Home. | *Work privately refers to care or services conducted outside of your agency or outside of scheduled approved shift. 14.Utilize gift cards or cash for member-related purchases, providing receipts for all purchases. | Take possession or utilize credit cards or ATM cards for member related purchases (see exceptions). Utilize member funds for personal purchases. | *Exceptions may apply on a case-by-case basis with prior approval of Kendal at Home. 15.Maintain confidentiality and follow HIPAA guidelines. | Share protected healthcare or personal information with others. | *Protected healthcare or personal information may include medical conditions, medications, financial information, and any other identifying information. 16.Maintain a smoke-free workspace, inside and outside of member’s home. | Request permission from member to smoke in designated area outside of the home. | *Smoke-free includes no use of tobacco products of any kind, vape, cigarettes, cigars, pipes, chew, and the like. 17. Maintain an odor-free, tidy car when transporting members. | Require a member to be transported in a vehicle that is not odor-free or tidy. | *Odor-free, tidy car refers to free of pet hair, trash, clutter, free of odors such as smoke and pets.
