Client Guidelines
SCHEDULING AND BILLI NG
It is imperative that any adjustments or shift cancellations be coordinated by our office and not directly with your
caregiver(s). All matters concerning you r billing and schedule should be communicated directly with our offic e
during regular business hours at 514 907 5065 .
OFFICE HOURS
Our office is open Monday to Friday from 9am to 5pm. We are on call outside regular hours (24 hours/day) for
emergencies ONLY.
CAREGIVER CLOCK -IN/CLOCK -OUT
We use an automated c lock-in/clock-out system that is based upon recognition of your home phone number. Your
caregiver will ask to use your phone to call a toll -free number when they arrive and when they depart.
The care giver will also report any mileage for transportation they have provided on your behalf using their
personal vehicle during the clock-out process. Mileage charges will be added as a separate line item on your
weekly invoices. The caregiver will also leave a detailed report summarizing their shift when clocking -out.
CLIENT BINDER /CARE NOTES
There will be a binder titled Home Care Assistance Care Notes left in your home . Your caregiver will note various
activities from their visit each day. These notes help to maintain a history of the activities and services provided
and allow easy reference for your case manager, medical professional and family. These binders and contents
remain the property of Home Care Assistance.
MONEY AND VALUABLES
Never give your care giver cash, credit cards, ATM/Debit cards, PIN numbers, che ques or other monetary sources.
Please make prior arrangements with our office if there is a need to have your caregiver make a purchase on
your behalf. We will provide the caregiver with instruct ions on how to make the purchase.
Never share your social insurance number, insurance policies, banking information or related financial information
with your caregiver.
Make sure that all of your valuables and personal information are in an easy to reme mber, discreet and safe
location.
If you have anything of exceptional or sentimental value please put it in a safe location (preferably locked) or
give it to a family member. If the item is prominently displayed in your home please instruct the caregiver not to
touch it.
Make sure you have an agreed upon location to store hearing aids, glasses, dentures or anything else of value in
daily use.
OTHER IMPORTANT INFO RMATION
- Our Care Manager, will, from time to time, conduct unannounced supervisory visits wit h our clients.
- We ask that clients have disposable gloves available for caregivers at all times.
We value our relationship and want to ensure that you are completely satisfied with the services we provide. If at
