LHA Business Rules - Canada Overview Living at Home Assessments (LHAs) are TheKey Canada's core clinical assessment tool used to evaluate a prospective client's care needs, safety risks, and recommended service plan. Scheduling Rules * LHA must be completed within 48 hours of initial inquiry (business days) * Assessments can be conducted in-person or virtually (virtual only with Regional Director approval) * Duration: typically 60-90 minutes * Must be performed by a certified Client Success Manager (CSM) or Clinical Lead Documentation Requirements * Full client health history and current medications * Home safety evaluation (fall risks, mobility barriers, kitchen safety) * Cognitive screening (standardized tool) * ADL/IADL assessment * Emergency contacts and advanced care directives * Photo documentation of home environment (with client consent) Pricing * LHA is complimentary for all prospective clients * No charge regardless of whether the client proceeds with care * If the client does not proceed within 90 days, a new LHA is required Follow-Up Protocol 1. CSM completes LHA report within 24 hours 2. Customized care plan is developed and shared with the family within 48 hours 3. If client accepts, care start date is scheduled within 5 business days 4. If client declines, CSM documents reason and schedules 30-day follow-up Exceptions * Hospital discharge assessments: may be expedited to same-day * Out-of-province clients: requires RDO approval for service area coverage * Clients with active Lifeguard subscription: priority scheduling Quality Standards * All LHAs are subject to quarterly audit by Clinical Quality team * CSMs must recertify annually on LHA protocols * Client satisfaction survey sent within 7 days of assessment
