Wellsky Deactivation Reason Dictionary


Accurate data starts with a common language. This dictionary is designed to help us move beyond the broad "No Longer Needs Care" bucket and identify the specific "why" behind every client departure. When our WellSky entries reflect the true story found in our care notes, we can better understand our clients' journeys and work together to improve the services we provide.

Reason
	Definition
	No Longer Needs Care
	Client has completely recovered and no longer needs any level of support (family, private cg or senior residency support included). Client has no long-term, chronic conditions that still require support. This may look like:
* Previously independent client recovers fully from hip replacement surgery
* Previously independent client no longer requires short term care after falling off a ladder
	Moved to Assisted Living/Memory Care
	This should be selected when a client transitions from Home Care to an Assisted Living Facility, Memory Care facility, or Nursing Home.
	Moved/Relocation
	Client is moving to a new city, province, or country. This includes those relocating within the same region if theKey no longer provides service in that area. 
	Admitted to Hospital/Rehab
	Client needs acute medical treatment or rehabilitation beyond what Home Care can provide. They are admitted, not visiting for an appointment.
May look like:
* Client has a stroke and needs 30 day rehab facility stay
* Client admitted for pneumonia treatment
	Cost/Affordability
	Client or family cites financial constraints as the primary reason. This includes:
* Fixed income clients who can no longer sustain the rate
* Family budget changes (job loss, etc.)
* Client exploring lower-cost alternatives  
This also applies when a client switches from theKey to a lower cost or more affordable option, or when there is a reduction in funded hours that makes service no longer viable.
	Funding Changes/Loss
	Government-funded hours reduced, benefits exhausted, or program eligibility lost. Key difference from Cost: here the client may want to continue but the funding source changed.
	Switching Provider/Caregiver
	Client chooses to leave theKey for another care provider or independent caregiver. This can occur due to dissatisfaction, referral to a competitor, or a personal caregiver connection.
	Dissatisfaction / Quality Concern
	Client expresses explicit dissatisfaction with the overall service quality, repeated scheduling issues, or general management concerns that lead to discontinuation of service. This is the catch-all for negative experiences not tied to a single caregiver.
	Family Taking Over Care
	Family decides to handle care themselves, often for financial reasons, personal preference, or when conditions stabilize. This is different from "No Longer Needs Care" because the care need still exists.
	Deceased
	Client has passed away.
	Seasonal/Temporary Hold
	Client pauses care for a planned absence (e.g., snowbird travel, family visit, summer months). They intend to resume services.
	Client Unresponsive/Lost Contact
	Multiple outreach attempts have been made but the client/family has not responded. There's no formal cancellation, just radio silence. 
	Minimum Hours Not Met
	Client's care needs are below our minimum service threshold. They may need help, but we aren't able to staff a case that small.
