SOP Fast Start of Care - Intake Team Workflow Applies To: LHAs Effective Date: 1/4/2026 (TBD) Purpose: To outline the required steps for Intake Team Members to process and activate Fast Start of Care opportunities for qualified clients. 1. Overview Fast Start of Care (FSOC) is an expedited intake pathway used only for qualified long-term clients (20+ hours/week). The goal is to accelerate onboarding while maintaining compliance, accuracy, and operational alignment. Intake responsibilities include client qualification, real-time local coordination, sending/collecting consent agreements, completing Salesforce data requirements, and transitioning opportunities to Closed Won for immediate SOC execution. 2. Eligibility Criteria A case qualifies for Fast Start of Care when ALL of the following apply: * Client commits to 20+ hours/week of ongoing care * Client is prepared to begin services within 24 hours * Local team confirms caregiver and schedule availability 3. LHA Workflow Steps Step 1: Qualify the Client * Confirm 20+ hour/week commitment * Verify urgency and start timeline * Confirm local team availability via Teams or phone Step 2: Notify Local Team * Contact GM or CSM to confirm caregiver match and availability * Get verbal confirmation before proceeding Step 3: Send Consent Agreement * Use TheKey e-sign platform to send consent and service agreement * Ensure client receives and signs within 2 hours Step 4: Complete Salesforce Requirements * Update all required fields in Salesforce opportunity * Change stage to Closed Won Step 5: Transition to Local Team * Confirm handoff details via Teams or email * Include client preferences, schedule, and caregiver assignment
