Veterans Affairs (VA) Go-To-Market PLAYBOOK VA Partnership Locations + Contracts: TheKey has a corporate executive partnership with the VA in select TheKey markets. VA Contract Statuses housed in shared folder. Reference Offices VA - Credentialed for active contracts. VA Homemaker Program: Available to all qualifying veterans enrolled in VA Health Care with clinical need for help with basic daily living activities. Homemaker and Home Health Aide is a trained person who helps Veterans take care of themselves and daily activities. Supervised by registered nurse in regulated states. Can be used with other Home and Community Based Services. Used as part of alternative to nursing home care and Respite Care. VA Community Care Program: VA provides care through community providers when VA cannot provide care needed. Based on eligibility requirements and needs. Long-term care services include: 24/7 nursing and medical care, physical therapy, help with daily tasks, comfort care, caregiver support. Settings: nursing homes, assisted-living, private homes, adult day health, veterans' own homes. Marketing + Enablement Tools: VA Flyer, VA Business Cards, VA Pinning Ceremony Strategy, Medal Honoring a Veteran Pins, Certificate of Honor. Tax ID and NPI Numbers confirmed with Regional Director of Operations. Approaching Stakeholders + Referral Partners: Clinic Prospecting: Find VA Location filtering for Facility Type: VA health and Service type: Caregiver support. Look for CLINICS within service territory. Visit Clinic - connect with front desk or Social Worker, ask for Community Health Nursing Team member. Community Health Nursing Team (PRIMARY STAKEHOLDER) within VA hospitals. They hold the list of home care providers and send authorization to TheKey. Key Stakeholders: Community Health Nursing Team (PRIMARY), Veteran's Service Office/Officer (VSO), Patient Aligned Care Team (PACT) Social Workers, Patient Advocate, VA Community Partnership (VCP) Group. Power Partners: AL/IL facilities (check if state allows VA Home Care Benefits in AL), Home Health and Hospice agencies affiliated with VA, Hospitals. LinkedIn Prospecting: Search local Clinic/Medical Center, filter by titles: Community Health, Social Worker, Patient Aligned, Patient Advocate, Primary Care Physician. Operational Process: The Ops Process Manual assists in creating and onboarding VA Clients into our systems and services. How Clients Qualify for VA Health Care Benefits: Must have served in active military, no dishonorable discharge, served at least 24 months continuous or full active-duty period, or discharged for service-connected disability/hardship. Need: Social Security numbers, military discharge info (DD214), insurance cards. Value Proposition - TheKey Honors Veterans: Pinning ceremonies for CSM team and Caregivers to honor veterans and families. TheKey Promise: Timely communication (available 24/7, callback within the hour 8am-8pm), Proactive communication (update VA on any status/condition changes), Utilization reports (weekly reports on HHA and respite benefit utilization), Care notes (indicate all ADLs and IADLs performed per authorization), Referrals (largest provider network, connect with trusted partners), Charity Volunteering, Education on VA-paid home care. VA Fee Schedule: 2024 schedule effective February 1st, 2024. Used by VA, TriWest and Optum. Procedure code G0156 billed in 15 min increments. One location uses S9122 billed in 1 hour increments. TheKey reimbursement rate through VA is on average 10-20% higher than top of home care rates. Salesforce Accounts: All VA Referral Accounts in credentialed markets roll up to site level Parent Account. Creating new VA Referral Account: Category = Community Resource, Sub Category = Veterans Affairs, Parent = VA Partnership site name, Partnership Type = Veterans Affairs. Aid and Attendance: Monthly payments added to VA pension for qualified Veterans and survivors needing help with daily activities or housebound. Eligibility 3-Ms: Military (90 days active duty, wartime, honorable discharge), Medical (non-service-connected condition needing ADL assistance), Money (net worth at or less than $150,538). Must need help with daily activities, or stay in bed due to illness, or in nursing home, or limited eyesight. War Periods: WWII, Korean, Vietnam, Persian Gulf.
