The ClearCare Care Plan is presented in the Caregiver app and will soon be available for family members in TheKey app. Do not input information that should not be shared with these parties.


Person-centered summary in Care Goals section of ClearCare: 


* 2-3 Sentence Client Description: 
[Client’s Preferred Name] is a(n) xx-year old [descriptors, e.g., woman, grandmother, former teacher, avid gardener] living with [most relevant conditions and/or symptoms, e.g., dementia, chronic pain, shortness of breath]. [Client’s Preferred Name] is looking for support with [Key areas of support, e.g., bathing, getting dressed, and getting to appointments]


* BCM Areas of Focus: 
Choose 1-2 pillars to focus on. You can refer to this list. Include individualized plan and/or barriers for each. Select the pillars and write at least one tip for implementation for each.

ADL/IADL Keys to Success in ADL/IADL section of ClearCare
*NOTE: MAKE SURE YOU ARE COMPLETING ADLs/IADLs in the right section in ClearCare. This ensures that your caregiver is seeing the tasks in the app. 
* Have at least one key to success for every ADL/IADL that is selected for that client (e.g., how to do it, what not to do, how to make it more enjoyable)
* Be sure to assign specific times to tasks (do not leave it as “all shifts, all days”) so Caregivers are directed to complete tasks at appropriate times during their shift (otherwise, they will be prompted to complete the task every shift, every day)


* At Least One Care Goal


* (Optional) Person-Centered Approach: 
* Include relevant goals and techniques from any of the appropriate supplements. 
Note: Do not paste in everything that is in the supplement; write only what is most critical for the caregiver and client contacts to know.
