Probation Notification – Amanda Steben Allard

Dear Amanda,

After your decision to quit your position as Scheduling Coordinator with Home Care Assistance (Montreal) Inc., based on a meeting between yourself and management on December 20, 2018, we have considered your subsequent request to withdraw that decision and to remain in your position with HCA.

We have decided to accept your request to remain with Home Care Assistance with the following conditions:

You sign the recent reprimand notice.  In addition, you will be put on probationary status for a period of ninety days from the date of this letter.  This also means that your performance review scheduled for January 2018 will be delayed until the end of the probationary period.  Any salary increase however, will be retroactive to your January 2018 anniversary date.

If during this probation period, termination of employment is desired by either party, no further obligation either financial or otherwise will be required.
