Effective __________, your client Louise Dontigny Quao residing at Chateau Westmount Square in Westmount has been admitted to hospital for an indefinite period.  As a result, your current work schedule with the client has been suspended; however, the client has agreed to continue to pay you based on your existing schedule in order to retain your services upon her release from hospital.

Should you choose to accept this arrangement, you must be available to return to work within four hours from the time the client is released from hospital.  Furthermore, upon her return home, you agree to continue working for the client for a minimum of four weeks from the date of resumption of service.  Should you not fulfill the conditions of this agreement, you agree to return the monies paid to you during the time the service is suspended.
