I have attached another version of the credit card form here as well – Please use whichever works best for you.
Moving forward, should we use Tim’s card for the entirety of the payment, or would you like to split up all the payments moving forward?

Hi Tim and Amy
Amy we need to change the credit card for payment. I have a lower limit and with the added amount, it will be difficult to ensure the payment goes through monthly, given our other operating expenses going thru. Tim has agreed to put it through on his end.
One payment of $5,266 on November 21 was put thru but the payment today for 2,266.80 was declined.
Tim, here is the credit authorization form that Corus requires for payment of the radio ads. If you fill it out, Amy can change the payment in the system.
My apologies, Amy, but this change will be necessary for future payments.
