They are asking for a "registration number if applicable" -- which I'm not sure is relevant for a PAB/PSW? I plan to leave this column blank. But what happens if the insurance company asks for a registration number? Please note that as a representative of Home Care Services, you will have to sign off on this form (as Maxine Silverman did 2 years ago) -- so that I can submit the claim to the insurance company for reimbursement.
