we have a similar set up to Tim's lifeguard in raleigh...its provided
through medicare/medicaid for clients so its not additional charge in
theory for them...happy to discuss more

RN Team,

Here is an example of feedback we get from Christian at VitalWatch to
update our care plans for our shared clients.  We had two instances with
Grady that our relationship with VitalWatch has saved him from a hospital
visit.

As of now, Mr. Ferrell is our only joint client and I know there are many
others who'd like to take advantage of the free service and give themselves
and family peace of mind.

Again, to qualify there needs to be a chronic situation and they have
Medicare or Medicare Advantage.  VW takes care of the rest and files
everything on behalf of the client.

Christian and I just met with Caitlin from Bayada at RMF and she loves
the concept and is going to push it with her clients that she feels could
benefit and we need to start doing the same with our clients.  This is not
only good as a retention/longevity/stickiness tool to keep folks in IL
longer and out of the hospitals and rehabs,  but also for new sales, as we
are in a highly competitive market.

Attached are fliers to review and as we onboard more dual clients, we will
need to also educate our caregivers of those dual clients as well, etc. on
an ongoing basis.

Please think on this and get me your list by EOD Monday if possible.

Christian and I are going to meet with Jan next and schedule a joint Home
Care and Remote Monitoring Information session at RMF to educate and sign
up and then take this on the road to other IL's we want to penetrate...
Atria's, Cambridges, etc.

Thanks Christian. I have updated his care plan and task list to encourage
frequent repositioning while he is in his chair and to encourage him to lay
on his side while in bed. I also added to encourage better hydration.
