Please have each individual attending the conference complete the registration form by* June 15th *so we can accurately project the number of attendees this year. *Caregiver of the Year Nominations:* At the conference we take time to honor those who are the heart of our company, our caregivers. We ask that each site nominate one caregiver to be considered for our *2016 Caregiver of The Year*. The nomination form is attached. We ask you to complete in its entirety. The winner will receive a free trip and one night hotel stay to the conference and will be honored at our first ever HCA Awards Dinner on Thursday evening. Please submit your nominations to Russell Gesher no later than *June 30th*. *Hotel Room Reservations: *The HCA discounted room rate of $199 per night can be reserved via phone or web. The discounted room block is available for the nights of September 6th-9th and the rates will be honored three days before and after those days upon availability.
