I will call them and ask them to set up. @Carolyn Daoust I can't see the CG name on the form. Do they provide any other form, because the garnishment form usually shows CG name and their address and the amounts and percentage. This form is little different from other provinces.This form is just explained how to deduct garnishment. Could you pls have a look? Because Ceridian will ask for that form to set up.
