The rational for our recommendation of 4 days per week with a shift of 5 hours was based on the fact that your mother had expressed and it was verified through our assessment, that she would benefit with support for both ADL (activities of daily living such as bathing and eating reminders) and IADL support (instrumental activities of daily living such as which include light housework and shopping). Given that the majority of these activities occur almost on a daily basis, continued support and ongoing cognitive stimulation would be beneficial. The shift does not include travel time. For example if the shift is from 8:00am until 1:00pm, the caregiver is to be on time and present for the entire duration of the shift. Our staff would be able to accompany your mother to her lunch and ensure that she is eating. Our staff are required to document everyday at the end of the shift how the day went and as well leave a verbal report on the phone to summarize how the day went. Families have access to the verbal report using the =E2=80=9CFamily Room=E2=80=9D and if any concerns arise, the care manager is made aware immediately. Therefore we ensure that there is daily physical activity, a healthy balanced diet, cognitive and social stimulation to enhance the overall quality of living. Our organization has made connections with additional services in the community. Therefore when a client is in need of a service that we do not provide, we are able to provide the appropriate referral with partners that we trust. Given your concern for your mother=E2=80=99s physical activity, w= e would be able to provide a referral to a physical therapist who would be able to create an enhanced exercise plan according to her needs. Our caregivers would then follow through with that exercise plan and assist her with any support she may require. In our caregiver charting notes, the caregiver would be required to document how she is progressing. The primary objective is to ensure consistent communication between the client, caregiver and care manager. Typically the first few weeks of care are a trial period for the family to get adjusted to the care plan and ensure that the caregiver the best match possible. Therefore we have no contract and for any changes in schedule, we only need 48 hours in advance. If you are aiming to have care 3 times per week, we would recommend spacing out the schedule. For example Monday, Wednesday and Friday from morning until
