Good Morning Sylvain,

My name is Allison Brennan and I am the new Care Manager assigned for your
fathers care. I understand you have had some issues with service lately and
I am very sorry to hear that and will do my best to rectify that.
As I am studying up on your dad's care, and learning more in depth about
his case, I would like you to know that I am your Main Point of Contact
moving forward.

If there is ANYTHING you need please contact me directly: *Work Cell

to use either of these numbers, I will answer.

I would like to set up a time to come out and meet you and get well
acquainted with your father and family.

Tomorrow is my community day and if you're available, I would love to come
out and do a reassessment to make sure that we are providing the
appropriate care that your father needs and have a conversation on how we
can improve the care moving forward.

This will give you the chance to meet me in person, and understand the way
I work. I am passionate about what I do and that reflects in the care I
provide to all my Clients, and
I'm happy that your dad will be well taken care of under my leadership.

I do apologize for all of the changes that have happened and lack of
communication, but rest assured that won't happen again.

I will be coming from Waterloo so I could be there around noon as I do have
to drop my son off to school for 9 AM and will leave from there after.

Please let me know if this does not work for you and we can arrange another
day and time that works best for both you and your family.

I have confidence we can work well together and have a smooth transition.

Of course, Cheryl will be available should you need to reach her as well,
but know that I am alway here to assist your family in any way that I can.

Please let me know if tomorrow works for you. If it doesn't, that's okay,
just let me know when and I will be there.
