I wasn’t aware this form was formally rolled out to your team but we received a completed form (emailed to the Help Desk) today. What is the expected process you have in mind when we receive this form that we can chat through?

I dug around and unfortunately no one seems to have the original Word version. I believe if you have a paid version of adobe you can convert it back to word.

Hi Steve,

 

Sorry to bother you again. I would like to create a waiver like this for all Canadian locations. In short, if a client received a government funded home care service (at their private dwelling) any additional care is considered exempt from sales tax. Should the service ever be cancelled, it would be the responsibility of the client to inform us of that change so we can reinstate the sales tax.

 

Can you assist with this?
