If we decide to apply for a new license we would need to take the following into consideration: Separate Office Location: If the second license is for a different geographic service area or office location, IDPH will require proof that the new location operates independently from the first. Distinct Operations: Each licensed location must comply independently with IDPH regulations, including separate: - Staff rosters, - Administrator or supervisor oversight, - Policies and procedures specific to the new location. No Duplicate Licensing for Same Location: IDPH generally does not allow two separate licenses for the same location unless there is a clear distinction in services offered (e.g., home services vs. home nursing). Agency Name Considerations: While the entity name can remain the same, ensure the name does not cause confusion about the agency's operations or locations. Some applicants add a suffix to differentiate between locations (e.g., "XYZ Home Services - North"). Initial Survey: - IDPH will conduct an onsite inspection to verify compliance with state regulations. On Thu, Nov 21, 2024 at 8:37=E2=80=AFAM Timothy Thomas wrot= e: > I see two options. > > 1.
