Hi Audra, I am pleased to inform you that in our current process, every office must submit their order between the 1st and the 10th of each month. With this, I already submitted the PPE list of orders for the month of April to our Vendor. Here's the link to our new & improved PPE & Test kit order process . Since Mavencare Toronto can't fill out the order form because they are not yet in the system, I have listed down the questions in the order form below for them to use in the meantime. Please fill out the questions and submit to me via email and copy Susie @ srichardson@thekey.com. Once the email order is submitted, it will be subject to approval and either me or Susie will confirm the final amount of PPE but this will already be added for the month of May. Please fill out the following for the PPE request. Requested By: Email: Office Address: How many active caregivers do you currently have in your office? Emergency COVID-19 Test Kits: How many active caregivers do you currently have working in a community setting that requires testing more than once a week? PPE: Vinyl Gloves (1 case = 1000 pcs) - Available in sizes Small,Medium,Large, and Extra Large: N-95 Masks (50 pcs per box): KN-95 Masks (per piece): Disposable/Surgical Face Masks (50 pcs per box): Face Shields (1 case = 116 pcs): Gowns (1 case = 50 pcs): Thermometers (1 case = 100 pcs): Emergency COVID-19 Test Kits (1 box = 2 pcs): If you are running low on supplies before the next purchase order, we encourage you to connect with your local medical supply stores and pharmacies (Like Amazon or Staples) to purchase PPE supplies and test kits. This is especially important as we will no longer have a place to store supplies. Last minute or individual requests will no longer be approved. Let me know if you have any more questions.
