Chris ---------- Forwarded message --------- From: Aretha Ilion Date: Tue, Feb 25, 2025 at 6:56=E2=80=AFAM Subject: Update on two tier one SOCs To: Cristal Munoz Cc: Christopher Reese Good morning, I just wanted to keep you updated on two tier one assessments that I scheduled and I believe would have started if the team would have been on top of things. On Friday I submitted a SOC for Chicago, daily, 8am - 8pm, SOC this Friday. I let them know via chat that this family was visiting with other companies and it would be best if they could get out Friday. The assessment was scheduled for Monday and the POC called yesterday and cancelled the assessment stating that she visited with another company and has decided to move forward with them. https://homecareassistance.lightning.force.com/lightning/r/006UH00000Kn6QYY= AZ/view The second Tier one came through yesterday and I have been working with this family since January. They were looking for 24/7 to begin ASAP. This client is in Grand Rapids where only Virtual assessments are completed. I sent the SOC email out and didn't get a response until three hours later. I checked my messages today and it states that they would not be moving forward because they have been in touch with two other services and are pleased with them. Mary Margaret was previously in AL at Clark but the staff and family noticed a cognitive decline around October or November and was moved to skilled nursing. During this time Mary Margaret has had multiple falls due to her attempting to get out of her bed and her chair. Kathleen stated that the family has not received an exact story as to what has occurred. Mary Margaret also has a history of chronic UTIs which the family hopes is the reasoning for her decline. She is actively being treated for this. Mary Margaret was also hospitalized due to a fall and was found to have fluid on her lungs and is on 2L of continuous oxygen. When in the hospital she was in the bed for one week and became very weak. The family would like to make sure that she is not removing her tubes as she has tried to do this in the past. I was told that Mary Margaret=E2=80=99s vitals and appetite is great= and the family would like for someone to take Mary Margaret to the dining area for meals. The family=E2=80=99s main concerns are safety and Mary Margaret rega= ining her strength so that she can move back to AL with her friends with 24/7 support. Please reach out to Kathleen as they would like to start care ASAP and continue 24/7 care
