There are some bad family dynamics between the three sons and facility that has resulted in the third party Silver Sherpa taking Legal action to decide what is best for the client. We are going to be there strictly for Observational purposes. What we are going to assess is the following: 1. How many times her sons call. Son #1 called between 15-25 times a day. The CG is going to document how the client is feeling/ what her demeanor is before the call, during the call and after the call. 2. How many times son #2 comes to visit. He is on a strict visitation schedule. He used to come 24 hours a day and sleep over with his mother.= He is in the mindset that when he is there, she doesn't have Dementia, and = is completely independent. Rhonda from Silver Sherpa has him on a strict schedule where he is only allowed to visit between the hours of 12pm-10p= m. He lives 2 hours away and comes everyday for all of those hours. He apparently was/is sleeping in his car in the parking lot of UG waiting f= or the hours he is allowed to go in. 3. Brother #3 usually visits 3x a week for three hours and causes no issues. 4. Brothers 1&2 are NOT allowed to be in the facility at the same time. There has been a HX of physical altercations with the brothers that have resulted in the DOC needing to call Police on both of them 5. We will need to keep a detailed account of all interactions the UG staff have with the client and how she responds to their attempts while providing personal care. We are to document strategies that do and do no= t work, and make sure we are thorough in what redirection tactics were use= d to get the client to comply. 6. How does the client interact with other residents in the facility? Can we get her to participate with facility activities and how does she handle them? 7. When she is interacting with her children, how was her emotional state before, during and after the interaction- whether it be on the pho= ne or in person.- was she angry and frustrated emotional? 8. DOS CHARTING- To provide objective data about Betty to determine any trends, patterns, patterns, contributing factors that are modifiable to navigate personal behavior's or expression. DOS needs to be complete wit= h all context of the situation at the moment. We are to document every 30 mins in the DOS Chart. 9. The Family does NOT think this client needs to be on a Locked Unit. This 2 week assessment will determine if the client is placed on the correct floor. 10. When she was D/C from hospital last, the hospital recommended that she needed to be on a locked unit as she was exit seeking- however, Rho= nda has mentioned that at no time has she seen Betty Exit
