GCM locations that are complete and have dba LiveWell filed with the state- - New York (dba's previously sent) - Florida (dba previously sent) - Oregon (dba previously sent) - California (dba attached) - Wisconsin (dba attached) - Texas (reinstatement and dba attached) In process - Massachusetts - Required notarization and paper filing. Takes about 3-4 weeks. ETA 1 week Canada New entity, LiveWell Care Management, Inc. - Our outside counsel is completing new paperwork to create this nationwide entity. Waiting on forms and will have them signed asap. Afte= r submission, I estimate it'll take a few weeks to process. I'll update yo= u as I learn more. Consolidating all Canadian entities (except LiveWell) under HCA Home Care Assistance Canada, Inc. - Our attorneys advise that the best time to do this would be at the beginning of the fiscal year. Michael in Tax agrees. I understand that t= he new fiscal year will start 1/1/2025. We are targeting this date to consolidate all Canadian entities. Note: LiveWell will still be a separate entity since it has different services. After consolidation, we will have a total of two entities in Canada.
