The first line is meant to be a signature line. We could revise it to say Signature of Client or Authorized Agent. I'm thinking it would be helpful to have a Printed Name line underneath the signature line since it's often difficult to read the signature and that can be especially difficult when it's someone other than the client signing. Then we can move "Self or Relationship to Client" to the right box. Thoughts?
