If you go to Reports/Custom Reports/Saved Reports you will see at the bottom of the list three reports with your name tied to them. The New Client Report and Caregiver Starts reports are very straight forward, just put in the timeframe you want to review and run the report for the data. The client report is also setup to show you the assigned Care Manager in ClearCare. The third report is setup the same as the client starts report. All you have to do is set the date range for the past 90 days. On a quarterly basis I think that it would be worthwhile to use the CCM Accountability Tool (attached) (FYI this tool is also available for ECM and BDM roles) to measure their success over the quarter. It is setup like the SER but just for CCM's and when you are ready to do them I can walk you through that process. One of the other things I would be reviewing with them is Tier A, B and C clients and their hours/revenue per week. This will be a way to start identifying those that you can move up in hours and those that you may want to start moving out. That can easily be pulled using the Gross Profit Summary Report and you can use the profile tags to show by CCM. Let me know your thoughts on that.
