In preparation for that day we are sending out some very important Pre-Work that will need to be *completed and sent back to the support team by Friday March 24th. * Here are the two things that we have for you and your team to complete: 1. The *Team Leadership Questionnaire* (attached) is the first piece that we would like you to complete and send back. In addition, we are asking that you please *send a copy of this questionnaire to each of your internal team members to complete* and send directly to James Patchett ( jpatchett@homecareassistance.com). The purpose of this questionnaire is to help determine your leadership skills from several perspectives. *It is crucial to our session that you and your team members complete this separately* and send the results directly to James. Please inform your team that the results will be kept confidential and to please be honest in answering. We will use the results to present you with a personal scorecard that will be used on Friday. 2. The *Goal Setting Document* (attached) should be completed for each of your locations separately and then sent to James Patchett ( jpatchett@homecareassistance.com) as well. You will also want to bring this to the session on Friday as we will use this throughout our workshop. The goal setting document is going to ask you for your 2016 actual numbers, 2017 year end goals, 2020 year end goals and 2022 year end goals as they relate to your core KPI's. We also have a section for you to write out your personal and business goals for the next year, 3 years 5 years and 10 years.
