On a weekly basis Sandra and your mother would do errands in the community (grocery shopping, pharmacy, etc.) as an effort to stimulate her, engage in some light physical activity and provide ongoing companionship. Furthermore, efforts were made to continue with her exercises, however as I mentioned to you in November, your mother began expressing to Sandra and myself that she was experiencing chronic pain in her legs and hips. The exercises that she was previously able to do became too challenging. At that point, we recommended a physiotherapist to reevaluate her situation. = As her condition was changing, we require ongoing involvement in the family to address the concerns. You are correct, the onus is on our staff to work with the client and overcome obstacles. We have made continuous efforts to achieve the highlighted goals however, =E2=80=9Csnapping out of a depression=E2=80=9D o= ften takes time and when there is only 12 hours a week with our staff, it leaves your mother isolated for the majority of the week. Though Sandra is not a qualified physiotherapist, she is a highly qualified and highly recommended caregiver. She has made a continuous effort to develop a relationship with your mother, be a supportive figure and continue to encourage her to exercise and eat well. Since December, I have expressed to you that she has felt isolated and lonely. It is very evident that her condition has continued to worsen. At that time, we recommended potentially increasing care as her chronic pain appeared to have worsened and she was feeling extremely isolated. Your response was to reduce care to one day a week. As it is not within our minimum requirement, we were not able to follow through with your request. There has been a lack of communication between our agency and yourself which has led to the issue escalating. The onus is on the family to be involved and address the concerns, we are only involved 12 hours a week which leaves your mother alone and independent for the majority of the week. I understand that you are not in Montreal however, there needs to be ongoing communication to avoid a crisis situation. The concerns for your mother=E2=80=99s mobility were highlighted numerous times by myself and later by the rehabilitation centre. The cancellation notice is only 48 hours however your mother contacted our agency to cancel services 1 week ago. Again, we made efforts to contact you and discuss options, however we did not receive a response. At this point, you are correct as it is a crisis situation. As I have suggested in previous emails, given her recent injury, your mother requires increased level of supervision as she is at a high risk of falling.
