To improve transparency and clarity for our clients, the new client packet will be separated by informational dividers to explain the documents in each section. The dividers are as follows: Our Agreement for Your Care - This section contains the main Client Consent Agreement and includes the key business terms the client cares about, including their rates. Our Terms and Conditions - This section contains the terms and conditions that govern the services we are providing to the client. Documents Required By Your State Licensing and Regulatory Agencies - This section includes documents required by the state licensing/regulatory bodies such as the client bill of rights. Documents Required by Your Insurance Provider - Finally, we=E2=80=99ve sepa= rated documents required by long-term care insurance providers. Care Estimate Survey - The Care Estimate Survey is now a stand alone document in the new client packet. The client will be asked to complete the survey to indicate their expectations for their care by using a simple drop down menu. Other than the dividers, the content of the CCAs remains unchanged other than some minor stylistic changes. The new version will be available in Pandadoc in the =E2=80=9CMy Templates=E2=80=9D section of Pandadoc moving f= orward. You will see all past versions of the CCA under =E2=80=9CDo Not Use=E2=80=9D. While we do not anticipate any issues with the launch of the updated CCAs, we have scheduled Pandadoc office hours from 12-1 PM PST on July 1st to assist with any troubleshooting.
