Good Afternoon Chuck,



Attached is your Amendment for the term July 21, 2024 to October 20, 2024.
Please review the Amendment carefully and if you are in agreement to the
terms and conditions set out therein, proceed as follows:



1.           Print a copy of the entire Amendment;



2.           Execute copy of the Amendment;



3.           Scan and return signed copy of the entire Amendment via email
to your Contract and Procurement Specialist and to
scss.centralcontractsadmin@gov.ab.ca within two business days;



4.           Please send originally signed Amendment via ordinary mail to
201, 4920-51 Street, Red Deer, Alberta, T4N 6K8



*Please ensure that updated proof of insurance (Insurance Certificate) is
submitted with your response, as it is a mandatory contractual requirement.*



A duly executed copy of your Service Provider Contract will be sent to you
after all Ministry approvals are in place.



If you have any questions or concerns regarding the terms of your Amendment
