Sunrise of Burlington hereby releases HCA from any medical liability relating to the services which are provided or are not provided by HCA, and Client acknowledges responsibility for the provision of any medical services. A general description of services to be performed is outlined below. Caregivers shall be capable of performing all typical Activities of Daily Living (ADLs), understand basic infection controls, proper lifting and transfer techniques, and have experience working with clients who have various degenerative conditions such as dementia, Parkinson=E2=80=99s disease, Multiple Sclerosis etc. Invoices are sent weekly and are due upon receipt. Client agrees to pay invoices on-time and understands interest charges may apply for balances over 30 days past due at a rate of 2% per month (effective rate of 26.82% per annum).
