Most importantly, the page should use the company logo. Your document is missing it and the government would think anyone can produce it. Please exclude payments for 2021 as the government would find it irrelevant and they would have to search for the 2022 data. It would be good to use "Date of Service" and "Payment Date" for two columns. I have paid the invoice for $128.00 regarding the service date of January 30, 2023. The email I received on Feb. 9 stated that the bill was past due; however I do not have the original email that was apparently sent before that. Kindly forward the original email sent prior to Feb. 9 to me for my records. I make it a habit to pay invoices promptly.
