Are you sure that Westminster is requesting all of this from us, as an agency? It makes sense if a resident is hiring a private caregiver but I am not sure they expect an agency to produce. Some of these documents * are our caregiver's HIPAA protected or privacy protected documents. If Westminster is requiring all of these documents, we will have to get permission from our caregivers to share.* (a) a negative Tuberculosis test - *not required for employment, so not in our files. . If Westminster requires this will they provide the test? If not, we will have to schedule caregivers to have one done - this will absolutely delay any SOCs and each one will cost us about $80* (b) a clean criminal background check - *we do background checks via IDPH quarterly and pre-hire we have an Accurate Background check is run - we do not share these reports.* (d) proof of COVID vaccination(s); * not required for employment - please ask Westminster to clarify what they are wanting. A booster in the last 12 months? A booster in the last 6 months? Again not required by us so this will delay or prohibit us being able to staff.* (e) required liability and other insurance, and, if the companion drives the Resident=E2=80=99s car on Presbyterian Homes property,* What insurance= are they requesting? The Key's Liability Insurance or caregiver's insurance? I can provide a copy of The Keys liability insurance.* (f) a valid driver=E2=80=99s license -* We have this on our files. Do they= want us to provide a copy of every caregivers deriver's license? We will have to get permission from our caregivers to share.* (g) appropriate auto insurance. - *We have this in our files. Do they want us to provide a copy of every caregivers auto insurance? Even if the caregiver is not driving their resident? **We will have to get permission from our caregivers to share.* Caregivers need to "Return Completed Form ("*Vehicle Registration - Third Party Personnel*") to the Public Safety Department along with Proof of Automobile Insurance (a copy of your state insurance certificate is acceptable)." * Do you know where The Public Safety Department is located? Do the caregivers have to make appointments? * Thank you, Diane On Thu, Jan 23, 2025 at 8:00=E2=80=AFAM Claudia Cook wrote: > Additionally, how are we handling these documents Westminster Place > requires (as listed on the 2nd page)? > > Presbyterian Homes requires that Residents who engage Third Party > Personnel > *providePresbyterian Homes with documentation* of the following with > respect to such Third > Party Personnel: > > (a) a negative Tuberculosis test, > (b) a clean criminal background check, > (c) a negative drug screen, > (d) proof of COVID vaccination(s); > (e) required liability and other insurance, and, if the companion drives > the Resident=E2=80=99s car
