If client does not request it will be up to the office to provide prior to start of care. *Recommended Future Solution:* For states that have additional forms to be provided at consent an "envelope" will be created in their DocuSign folder that the Web Lead Team can send along with consent. *Dependency: *Here again we have no visibility to what states have these added documents and what those forms might be. Great news here is we will not get visibility to better understand.
