If the team member does not intend to pay their portion of the premium cost/wants to opt out they must provide written notice. The applicable benefits will then be suspended (the insurance carrier may require further evidence of insurability prior to reinstating the team member=E2=80=99s ben= efits coverage upon the expiry of the leave). On the other side of it - Any team member who wishes to enroll their new born as a dependent on their benefits plan must do so within 30 days of the birth.
