Do we have a direct deposit form for JP Morgan? I am trying to have Medavie
Blue Cross deposit directly into that account instead of the SJD account. I
also need a copy of the SDJ business registration. I no longer have that
but Blue Cross requires it to make this change.

Blue Cross. Some of our clients have their coverage and are finding it
difficult to pay for our services upfront. In order to direct bill though
we have to be on their registered vendor list and they require certain
information for that. Firstly, I need the business registration number for
Ontario and secondly I need a direct deposit form for the JP Morgan account
so the money can be deposited in there automatically.
