I spoke with Tim and one main objective of October will be tracking the assessments that close with home care and Lifeguard vs the assessments that only close with HC. The goal is for each client to move forward with HC & LG, as it is included and encouraged by TheKey in order to provide the best home care. LG focuses on helping the individual to live longer, independently in their home. During the time that a caregiver is not there, LG will be the clients safety net. How many assessments have been completed in Oct so far by the CM's? What will be the best way to start the conversation now that the assessment is complete? I was thinking that if we can have the CM's email the contacts an intro email (I provided a template below) maybe that would be best? What are your thoughts? And I could reach out after. Also, we will need to track these assessments. I have added a new tab in the current client list excel. Please add all new assessments there. I will bring printed versions of the brochure so the CM's can leave them at the assessment.
