Here is what I'm proposing. Let me know your thoughts. [image: image.png] Elevate Hospice (Shows up as Specialized Home Care and Hospice in SalesForce - they were bought and do not have home care) - Replace Catholic Community Hospice who has an ADC of around 30. Elevate's is 250 and they are the ones I've been meeting with. The Princeton (IL/AL/Memory care - not in SF) - replace LC Center of Osawatomie as they are in the process of closing if they have not already. Solstice Senior Living at Lee's Summit (Independent only -Not in SF) - Replace Villa Ventura as it closed last year. In all honesty I don't know very much about The Princeton or Solstice, but it looks like they have never been called on and have potential.
