Hi Audra,

Just to clarify, this is specific to the US lines of business that we bill using the CMS 1500 form. Currently, we are not billing any Canadian lines of business on 1500 forms, but it's not a bad idea to have this on file for those clients as well.

  Tri-west sent us notification that they will be auditing our charts to ensure we have an AOB on file for all VA clients. As a result we will need to have our VA, WC, and No fault clients sign a release, allowing us to share their information with the payor, as well as authorizing us to receive the payment directly. We will need to move quickly in getting all signatures in order to be in compliance. *	Printed Copies: Available for wet signatures when needed

Step-by Step Process:

 

At Client Intake:

*	Ensure the client (or their authorized representative) signs the AOB form.
*	Explain the purpose of the form to the client for transparency.
*	Verify that all required fields are completed.

For Existing Clients:

*	Identify VA, WC, and No-Fault clients without a signed AOB on spreadsheet.
*	Contact these clients to complete the form promptly.

Upload:

*	Upload signed forms to the clients Wellsky profile.

Completion Date:  December 31st
