I would like to introduce you to your Client Success Manager (care manager), Tera McDuffie, who will be your primary point of contact as we begin services. Her role is to help you navigate your home care needs, keep you updated on and manage the care schedule as well as to ensure we have a great care team in place. We are confirmed to start care tomorrow, July 6th. Tomorrow's shift will be 10am-2pm as a cross-training shift with Paula. Tera will be at the start of care tomorrow with your caregiver Mary. As discussed, our on-going schedule starting Friday, July 7th will be 8am-4pm daily. We also discussed scheduling care for 30 days to ensure caregiver consistency. However, Tera will continue to assess your needs during those 30 days and will assist you in ensuring we have the most appropriate care schedule that will meet your needs. Your hourly rate based on your current level of care is $37/hr. Tera will also collect your long term care insurance policy information tomorrow so that we may assist you in the claims process. Here are a few important additional details to remember: - Be mindful of our 48-hour cancellation policy. We require 48 hour notice to cancel or adjust your care schedule, otherwise you are liable = for the full cost of the shift. - We are 24/7/365. Our 24 hour number is listed below. - Any schedule adjustments must be made with your care manager or through our office. Our office number is listed below. - We have a dedicated long term care insurance department that will guide and support you with the long term care process. We pride ourselv= es in ensuring your long term care insurance reimbursements occur timely. - If/when applicable overtime is billed after the 8th hour of service in a single shift. The overtime rate is time and a half of your hourly rate= .
