She resides alone at *1749 Bloor St, Apt 204, Mississauga, ON L4S 1X6 *and can be reached directly at 416-400-9322 or 416-801-8488. Sara has a dual diagnosis and is struggling with hoarding, as well as general mental health and trauma. She recently underwent back surgery and has endured extreme weight loss. Sara was a head nurse at CAMH for 30 years, but during her career, she was attacked by a patient, which has led to memory loss, PTSD, anxiety, and depression. Unfortunately, Sara is not eligible for EI or OW. She recently began receiving her pension, but as you are aware, pensions are often not sufficient to meet basic needs. She is currently in arrears for five months of rent, totaling approximately $8,500, for her two-bedroom apartment. Given her complex needs, we are seeking assistance with the following: - *Home-based comprehensive assessment* with a geriatric psychiatrist (e.g., at home, retirement home, or long-term care facility) - *Consultation, education, and care coordination* for patients, families, and caregivers - *Collaboration with family physicians, other health care providers, and community partners* to facilitate linkage to support services - *Development of behavior management strategies* tailored to her needs - *Short-term supportive counseling and health teaching* for both Sara and her family - *Assessment by a nurse practitioner* for seniors with complex mental illness, particularly for those who are frail, have multiple health conditions, and frequently visit the Emergency Department I am a single mother of a 14-year-old son with ADHD and ODD, which limits my ability to support Sara fully.
