The client consent, the emergency contact form, the credit card authorization form and the direct deposit form. We however do not have the authority to upload our new client consent form so we need that to be uploaded. It is attached here. Please note that this document needs the ability to be changed each time we send it out so can it be uploaded in such a way that we can manipulate it?
