When we want to get details of everything a client is doing (or not doing) on an overnight shift our care manager asks for written details for specific time periods in the care plan then discusses it with the caregiver. The caregiver then documents accordingly in the existing everyday care notes. They then take a picture of the care note and text to the care manager at the end of the shift. We don't do this every day but are getting more and more requests from care managers for these details. We are looking into an app for digitally sending in care notes.
