The letterhead is not necessary – this is only a suggestion as it is in the CNESST template.

Hi Florence,
Just to be clear, do you need both English and French resolution or just the French one signed? Also, why would a resolution need to be on a letterhead? Can't we just use the title of the resolution "Resolution of the Board of Directors of Home Care Assistance (Montreal) Inc." like we do on all our previous board resolutions? Please confirm and I will get that out for electronic signature. Thanks.

1) An attestation from Revenu Québec
2) A resolution from directors authorizing Tim Thomas to apply for the license, and
3) Add an extension number to the " respondent " section of your application.
They are asking that these documents be provided within 10 days of November 2 (date of letter) otherwise a process of refusal of the application may be initiated.
After correspondence with Tim last week, I understand that these documents can only be submitted by mail.
We obtained the attestation from Revenu Québec for you last Monday. I also sent you a draft resolution for signature last Monday, but have not yet received a signed copy.
As for the extension number, Timothy explained that you do not have one.
I will be able to send the attestation from Revenu Québec and the resolution (the missing documents) by mail as soon as you send me a signed copy of the resolution.
