 Just touching base here, I still do not show an updated consent agreement with the needed missing information filled out. Please let me know if there is an update that can be provided.  
   
   
 We audited this new Chicago client and discovered the person who signed the consent agreement is not listed under the financial party section of the agreement. Are you able to please assist in securing an updated consent agreement that lists Michael under the Financial Responsible Party section? 
   
 This is important since Michael's name is not listed on consent agreement and they signed, so we cannot hold Michael to the terms and conditions of the agreement. Vice Versa, Judiths name is listed on the consent agreement but they did not sign the agreement, so we cannot hold Judith to the terms and conditions either.
