We began providing care for Ms Angela Rogers (apt# 709) back in January of 2017. We placed a caregiver by the name of Lynette Gunpat (picture attached) with her for a period of 4 months up until early April. Care was subsequently cancelled as the family informed us that Ms Rogers had improved and no longer required care. Upon visiting the residence this past Wednesday to complete an assessment with Ms Shade, I observed Lynette entering the apartment of Ms Rogers. I knocked on the door and very politely asked to speak with Lynette. Ms Rogers told me that there was no caregiver present in the apartment. I said that I had observed Lynette entering and that if she could pass along a message to her that Timothy from Home Care Assistance will be contacting her shortly. I did everything possible not to stress Ms Rogers but I could tell that she was uncomfortable. She likely knew that she was in breach of our contract. I've attached the contract that the Rogers family signed. As you can see on the first page under "HCA Caregiver Agreement", clients are prohibited from hiring our caregivers privately. Also attached is the agreement that Lynette signed which has much of the same language. Given the great relationship we have with you and the R=C3=A9seau West Isla= nd, I would like to ask permission to pursue this matter directly with the family / caregiver. With that being said, I don't want to cause you or the R=C3=A9= seau any unnecessary discomfort or annoyances. Please review this matter with your director and let us know your thoughts. I'm available to come in and discuss this in more detail if needed. At the very least, I would avise you that Lynette is not a trustworthy caregiver and she is most likely soliciting business with other residents in your building.
