Please contact the caller to introduce yourself and confirm the assessment date/time. **Please confirm the person completing the consultation. ** Assessment Date and Time: 1/20/26 at 11am CST Client Name: Lynn Whittington Chatter Notes: COUPLES CARE: Dad is 87 and mom is 86. Dad has had slow, cognitive decline over the last 6 years and is independent of all ADLs so may need help with shaving. Is starting to have hallucinations but has never been combative. Daughter sees a lot of fear when he hallucinates and when this happens, he goes next door to his neighbor. Client is definitely a flight risk. Daughter lives about 25 minutes away and neighbors can help if there's immediate need but the neighbor is also elderly. Mom has scleroderma with Crest syndrome - autoimmune and eventually, all of her soft tissue in her organs will harden and she won't be able to breathe. Progression has been slow. Both mom and dad are losing weight but mom has lost weight dramatically. Mom uses a walker to ambulate but she is unsteady. If you try to help her, she will brush you off. Has a Hx of falls and broke her hip once and another time, she broke her arm right below her shoulder. Attends to all her ADLs but has some urinary incontinent but uses Depends at night and absorbent pads during the dad. Mom is a proud woman, according to daughter, and wants everyone to believe that things are normal. There are 3 daughters. Mary Beth, caller, is an RN and POA for both mom and dad. Mom's cognitive status has been declining rapidly b/c she's in the cg role and doesn't sleep well. Also has a hearing issue and needs to be evaluated. She's not getting the cognitive input she needs. Mom and dad sit in the kitchen all day and count how many times the UPS truck has driven by, watch the neighbors walk by, etc. Needs cg with dementia/Alzheimer's experience who knows how to de-escalate since mom doesn't know how to do this. Lynn can be a "prickly pear" according to Mary Beth, daughter, b/c she wants to remain independent and may resist care at first. Needs for dad: Socialization, physical engagement, med reminders, encouragement to eat their Meals on Wheels meals, cooking/meal prep Needs for mom: Med reminders, standby assist in the shower, cooking/meal prep Schedule: M-F for at least 8 hours and will eventually go to 7 days a week SOC: ASAP CG preference: Versed and experienced in Dementia/Alzheimer's care Quoted hourly rates, minimums, and confirmed private pay.
